Yesterday, March 16th was World Social Work Day! Hospice of Hope’s Medical Social Workers are a VERY important and appreciated part of the interdisciplinary hospice team. Everyday they are faced with different challenges and work endlessly to make sure their patients and families are taken care of. They truly give their all to our community with countless hours of counseling, connecting families with community support systems, helping navigate goals of care, and much more. Our Social Workers seek to prepare families and patients with what to expect during hospice care and continue to follow up with loved ones for several months.
