RN Case Manager - Ohio Team- Full-time, Monday-Friday, 8 am to 4:30 pm, with weekend on call rotation as scheduled.
The Nurse Case Manager provides compassionate nursing care for terminally ill patients in their homes in Kentucky service area. Graduate of an accredited School of Nursing and current eligibility for RN licensure in Kentucky and Ohio. At least one-year experience in a patient care position in a hospital, nursing home, or related health care facility with demonstrated knowledge of end of life or palliative care preferred. Personal qualities of empathy, responsibility, and dependability. Must demonstrate effective communication skills with team members, providers, patients, and family members. Must have ability to work independent of direct on-site supervision.
Driver's license, auto insurance, reliable transportation, pre-employment criminal background check and drug screen required.
