Hospice of Hope Launches Advanced Cardiac Care™ Program in Collaboration with American Heart Association and National Partnership for Healthcare and Hospice Innovation

Hospice of Hope, in collaboration with the American Heart Association (AHA) and the National Partnership for Healthcare and Hospice Innovation (NPHI), is proud to introduce the new Advanced Cardiac Care™ (ACC) Program. This initiative addresses heart disease, one of the nation’s leading causes of death and brings critical advanced cardiac care services to residents of [insert geographic location].

The program includes a patient and caregiver guide that provides practical tools and compassionate guidance to help families manage heart care at home—reducing unnecessary emergency room visits and easing the strain on both families and healthcare systems.

Heart disease remains the leading cause of death in the United States, claiming more lives each year than any other condition and resulting in more than a million hospital admissions for congestive heart failure annually. Through NPHI’s national network of nonprofit providers, patients can access comprehensive medical, emotional, and lifestyle support—often covered by Medicare or private insurance—to help them live more comfortably at home.

An important goal of the guide is to ensure that all individuals, especially underserved groups, are better able to access important care resources while experiencing advanced heart disease.

“Heart disease disproportionately impacts underserved communities, including people of color and LGBTQ+ individuals, who often face higher rates of related risk factors,” said Dr. Cameron Muir, Chief Medical & Innovation Officer at NPHI. “Many living with advanced heart disease aren’t aware of the support available through NPHI members. Our goal is to bridge that gap—ensuring every patient, regardless of background or income, has access to care that improves comfort, dignity, and quality of life.”

NPHI represents nonprofit hospice and advanced illness care providers nationwide. These providers offer comprehensive medical and healthcare at home, extending support even to non-terminal heart failure patients.
