The Transitional Care Nurse is responsible for coordination of care for a chronically ill, high-risk patient population. The Transitional Care Nurse provides follow-up, support and linkage to community resources and providers for individuals and families, based on an ongoing assessment of need. The Transitional Care Nurse is responsible for evaluating patient progress with the plan of care. Graduate of an accredited School of Nursing and current RN licensure in the state of Kentucky and Ohio. At least one year of experience in a patient care position in a hospital, nursing home, or related health care facility with demonstrated knowledge of nursing home rules and regulations preferred. Personal qualities of empathy, responsibility, and dependability. Must demonstrate effective communication skills with team members, providers, clients and family members. Must have ability to work independent of direct on-site supervision. Valid driver's license, car insurance, reliable transportation, pre-employment criminal background check and drug screening required.
Shift time: 8 am to 4:30 pm, Monday through Friday, with weekends and on-call hours as scheduled.
